We look at the member ID, the drug, the diagnosis the clinic provides, and the plan’s rules. The same injectable can be a pharmacy-benefit specialty fill one month and a medical-benefit infusion the next, depending on the payer. That difference decides whether prior authorization goes to a PBM portal or a medical-authorization team.
